About Vision Home Health & Hospice Care

 

Vision Home Health Care, Inc. was established in 2002. In 2003, Vision Home Health Care, Inc. became a Home Health Care Provider and State Licensed Medicare and Medi-Cal Certified agency, organized to provide professional health care services throughout the Inland Empire, with its office located in Riverside, California.

In 2008, Vision Home Health Care, Inc. was designated as a Home Health Care Elite provider — a listing of the top 25 percentile of home health agencies in the United States — by Outcome Concepts Systems.

 

The agency outgrew its original location and relocated to a larger office in 2012, where services areas expanded to some parts of surrounding counties including San Bernardino County, Orange County and Los Angeles County. The organization prides itself in the high level of quality care provided by a well-qualified multicultural staff. 

 

In 2014, the agency adapted to provide hospice services as well and became Vision Home Health and Hospice Care, Inc. and achieved the Gold Seal of Approval from the Joint Commission on Accreditation of Health Care Organizations.
