Who is Eligible for Home Health Care?

Home health care is provided to patients who are in need of intermittent skilled care and leaving the home requires considerable taxing effort. Home health clinicians provide the same level of skilled care and rehabilitation services the patient would otherwise receive in another health care setting such as a hospital or nursing home. These services are ordered and directed by a qualified physician who coordinates the care of the patient directly with the agency.

 

Examples of common conditions that may qualify the patient for home health include wound care for pressure sores or a surgical wound, patient and caregiver education, intravenous and nutrition therapy, injections, monitoring serious illness and unstable health status. Home Health Care Coverage

Based upon individual situations and services required, your home health care may be covered by Medicare, as well as most HMO and PPO insurances. Your doctor or hospital may refer you to home health care services. As the patient, you have the right to choose the agency that will be providing your home health care services.

 

Services Offered

Skilled Nursing Registered Nurses and Licensed Vocational Nurses provide instruction, treatment and coordination of health care.

 

Medical Social Services Medical Social Workers and Licensed Clinical Social Workers assist with referrals to community resources and provide counseling related to the impact of illness or long term care.

 

Rehabilitative Therapy Physical Therapists, Occupational Therapists and Speech Pathologists provide treatments designed to develop and restore muscle and joint function, improve self-care abilities and/or communication skills.

 

Certified Home Health Aides provide assistance with activities of daily living for the patient while they are receiving nursing care and/or therapy services.
