Who is Eligible for Hospice Care?

Any patient with a limited life expectancy, whose desire is to have a physical, emotional and spiritual comfort, may qualify for hospice care. The patient's primary physician and the hospice medical director must authorize the start of hospice care. 

 

Patients with a wide range of diagnoses can qualify for hospice services if the disease is in its advanced stages. Examples of qualifying diagnoses include, but are not limited to, cancer, dementia, chronic lung disease, heart failure, stroke, and chronic kidney disease.

 

Hospice Care Coverage

Vision Home Health and Hospice Care, Inc. is a Medicare/ Medi-Cal certified program which accepts patients without regard to age, gender, nationality, race, creed, sexual orientation, disability, diagnoses or availability of primary caregiver. Hospice services are covered by most insurance policies. No one is refused services because of inability to pay. Admission to hospice is based on the patient's eligibility and need.

Hospice care is patient-centered and patient directed. It is one of the best examples in health care of collaboration by an interdisciplinary team, dedicated to meeting the needs of the patient and family. Our team works closely with the patient's family and physician to meet the patient's needs in the comfort of their own home, or in the residence of their choice. 


Because dealing with the death of a loved one can be a difficult time, we offer grief support for families dealing with their loss, helping to guide them through the grieving process. Support includes follow up calls, bereavement counseling and support groups.
